Step 1: Cook the Pasta
Bring a large pot of salted water to a rolling boil. Add the fettuccine and cook according to package instructions until al dente. Reserve about 120 ml (½ cup) of pasta water, then drain and set aside.
Step 2: Brown the Sausage
Heat olive oil in a large skillet over medium heat. Add the sausage and cook for 6–8 minutes, stirring occasionally, until browned and cooked through. Transfer the sausage to a plate and set aside.
Step 3: Melt Butter and Sauté Garlic
In the same skillet, melt the butter over medium-low heat. Add the minced garlic and sauté for about 30–45 seconds, just until fragrant. Be careful not to let it brown, as garlic can turn bitter.
Step 4: Build the Sauce
Slowly pour in the heavy cream and milk, stirring constantly. Let the mixture warm gently for 2–3 minutes, until it begins to steam and thicken slightly.
Step 5: Add Parmesan
Gradually stir in the grated Parmesan cheese. Continue stirring until the cheese melts completely and the sauce becomes smooth and creamy. Add salt, black pepper, and Italian seasoning if using.
Step 6: Return the Sausage
Add the cooked sausage back into the skillet. Stir gently to coat it in the sauce, allowing the flavors to meld together for 2–3 minutes.
Step 7: Combine with Pasta
Add the cooked fettuccine to the skillet. Toss everything together until the pasta is evenly coated in the creamy garlic Parmesan sauce. If the sauce feels too thick, add a splash of reserved pasta water until it reaches your desired consistency.
Step 8: Finish and Rest
Remove from heat and let the pasta sit for 1–2 minutes. This allows the sauce to settle and cling beautifully to the noodles.
Step 9: Garnish and Serve
Sprinkle with fresh parsley, extra Parmesan, or red pepper flakes if desired. Serve immediately while hot and creamy.
